Reinstate a Contractor's License

If the contractor license has been expired for more than one year, the license holder must complete a new application and submit supporting documents as required for a new license.

In order to retain an existing license number, reinstatements must be completed in person or by mail, not online. Once reinstated, the contractor's license number will remain the same.

Expiration dates are listed on each license and on the city's licensed contractor search page.

Note: If you are reinstating a right-of-way (ROW) or sewer license do not follow the below steps. Go to the ROW licenses information page to learn how to reinstate your license.

By Mail

Step 1.Complete Application

Fill out the contractor license application(PDF, 153KB). Note: Illegible or incomplete applications will not be accepted.

Step 2.Supervisor Statement of Responsibility

The supervisor certificate holder must fill out a Supervisor Statement of Responsibility(PDF, 125KB). If the supervisor certificate holder is not a company owner, officer, partner, member, or sole proprietor, the supervisor must submit a letter on company letterhead stating they will be the “Supervisor of Record” for the company for this license. The letter must be signed by an owner, officer, partner, member, or the sole proprietor.

Step 3.Photo ID

Submit a copy of the supervisor certificate holder's photo ID.

Step 4.Secretary of State Documents

Compile these Secretary of State documents according to the legal status of the company.

Step 5.Complete Affidavit of Lawful Presence

As required by state law, please fill out and notarize the Affidavit of Lawful Presence(PDF, 119KB) and include a notarized copy of your “Secure and Verifiable Identification” as listed on the form.

Step 6.Submit by Mail

After completing the above steps, compile the required documents and payment and mail them to:
