The five medical plans offered to eligible City employees are Denver Health Medical plan (DHMP), Kaiser Permanente HMO, Kaiser Permanente Deductible HMO (DHMO), United Healthcare HMO and United Healthcare Navigate Plan. Medical, Dental, Vision, Short-term Disability and Flex Cash are effective the first of the month following your date of hire. Your life insurance and long-term disability become effective the first day of work.

Denver Health Medical Plan (DHMP)
As a member of DHMP, you receive your care through the doctors at 10 community health centers throughout Denver.  All locations have free, easily accessible parking for DHMP members. Several convenient pharmacy options are available, including online prescription refills and Pharmacy Delivery by Mail, offering a 90-day prescription supply for one co pay. To learn more about DHMP visit www.denverhealth.org or call member services at (720) 956-2100.

Kaiser

Kaiser Permanente is the largest nonprofit health plan in the United States.   They are an integrated health delivery system, which means they provide and coordinate the entire scope of care for members, including: preventive care, well-baby and prenatal care, immunizations, emergency care, screening diagnostics, hospital and medical services, pharmacy services. United Healthcare Choice Plan (HMO/EPO)
The UnitedHealthCare HMO/EPO plan is similar to a traditional HMO which coordinates your care through a primary care physician and allows you to pay a low flat fee (copayment) for most covered services. To find out more about UnitedHealthCare Choice Plan, visit https://www.myuhc.com/member/prewelcome.do or call 1-800-842-5520.

UnitedHealthCare Navigate Plan
Members enrolled in this plan have access to a network limited to Colorado.  In exchange for lower premiums, members will be required to pay a deductible and coinsurance for services.

Important:  If enrolled in Navigate, you must choose a primary care physician. In order to see the doctor of your choice, email ccdnavigate@uhc.com with the 11 digit Provider ID in the 'Navigate/Navigate Balance' plan found by clicking 'Find Physician, Laboratory or Facility' at https://www.myuhc.com/member/prewelcome.do
