Long Term Care
The Long-Term Care Program (LTC) has several program services. These are:
There are several requirements, which are universal to all categories of assistance.
Residency Requirements:
- A Colorado address must be established.
- Application must be made in the county of residence.
- There is no time of duration requirement.
Citizenship Requirements:
- Applicant must be a United States or naturalized citizen or a legal alien admitted to the United States.

Requirements and Benefits
Long-Term Care
Requirements:
Applicants must be:
- Institutionalized for 30 consecutive days;
- Certified by a Colorado licensed doctor as being disabled for 12 months. If applicant is 65 years or older, they will meet the disability requirement without certification.
Benefit:
Home Care Allowance
Requirements:
- Income must be under the new basic grant amount. To see if you are eligible for this and other programs, click here to go to the Benefits Check-Up page.
Benefits:
- Provides in-home services for daily-living type care, such as: cleaning; personal care assistance (bathing, dressing, etc.).
- Medicaid is a benefit only if the program the HCA is attached to has Medicaid.
- If HCA will be attached to State AND (Aid to the Needy Disabled), there will be no Medicaid coverage.

Home and Community Based Services (HCBS)
Requirements:
- Applicant must have received HCBS services for 30 consecutive days.
- Applicant must be certified as being disabled for 12 consecutive months (being 65 years old or over meets the disability requirements).

Benefit:
- Provides in-home, medically necessary services from medical professionals.

Adult Foster Care (AFC)
Requirement:
- Applicant must have Supplemental Security Income (SSI) or Old Age Pension (OAP) and be in an Adult Foster Care facility.
Benefit:
- Medicaid coverage and the facility provides daily supervision.

Alternative Care Facilities (ACF)
Requirements:
- Applicant must have been in the facility for 30 consecutive days.
- Applicant must meet a 12 month disability requirement (65 years or older meets the disability requirement).

- Medicaid coverage and ACF services.

Nursing Home (NH) Institutionalization
Requirements:
- Applicant must have been in the facility for 30 consecutive days.
- Applicant must meet a 12 month disability requirement (65 years or older meets the disability requirement).
