 DenverWellness Incentive Program 2013 Minimize

Get Healthy, Enjoy Life, and Now Save Money!

Starting on September 1, 2011, through August 31, 2012 employees will be eligible for a wellness credit  on their pay checks in 2013 by completing the actions of the City's DenverWellness Incentive Program.

In an ongoing effort to promote employee health and well-being, and reduce health care costs for employees and the City, three actions are required:

  1. Complete the online health assessment through your city sponsored health care provider (Denver Health, Kaiser Permanente or UnitedHealthcare).
  2. Complete one Physical Activity Program of your choice:

    a.   Option 1 - Complete one DenverWellness worksite activity program:


    b.   Option 2 - Complete the Self - Paced Physical Activity Challenge:

    • Complete a minimum of 30 minutes of continuous aerobic activity, at least four days a week
    • Log weekly activity for the entire 8 - week challenge (brisk walking, bicycling, running, swimming, basketball, hiking, etc)   
     
  3. Complete Nutrition Challenge

                 a. Healthy Plate     

  •  Log at least 5 Healthy Plates every week

 Online Health Assessment Minimize
We invite you to take the free online health assessment, created in collaboration with our health care partners UnitedHealthcare, Denver Health and Kaiser Permanente. It's free to you, and only takes about 20 minutes to complete.
 
The questions are designed to provide you with a detailed, personalized health report which your health care provider will send back to you, showing the relationship between behavior and healthy habits, today and your healthy future. All information is confidential between you and your provider.
 
To get started with your online health assessment, we recommend knowing your 4 key health numbers.  Your 4 key health numbers, cholesterol, blood sugar, blood pressure and body mass index (BMI), are used to complete your online health assessment.
 
The online health assessment is one of the required actions of the DenverWellness Incentive Program and must be completed to be eligible for the wellness credit.

 Self-Paced Physical Activity Challenge Minimize
Challenge Description
The Self-Paced Physical Activity Challenge is an 8-week challenge that encourages employees to pursue a healthier lifestyle by being more physically active. Employees log their weekly aerobic physical activity and aim to complete at least 30 minutes of aerobic activity 4 days each week. The challenge can be completed anytime between September 1, 2011 and August 31, 2012.
 
Challenge Requirements
To be eligible for the wellness incentive, participants must complete a minimum of:
  • 8 consecutive weeks of aerobic activity
  • 4 days each week
  • each day must be at least 30 minutes of non-stop aerobic activity
  • log their weekly activity on the designated challenge log sheet for the entire 8 weeks
Aerobic, or cardiovascular, activities must be continuous and performed at a moderate-intensity level. According to the 2008 Physical Activity Guidelines for Americans, moderate-intensity aerobic activity means you are working hard enough to raise your heart rate and break a sweat.

 Healthy Plate Minimize

Challenge Description

The Healthy Plate Nutrition Challenge is a new DenverWellness program that encourages employees to eat well balanced meals that contain all five food groups. Participants will use the USDA's MyPlate guidelines to build five healthy plate meals each week. The challenge can be completed anytime between September 1, 2011 and August 31, 2012.  

Challenge Requirements

To be eligible for the 2013 wellness incentive, participants must complete a minimum of:

  • Log at least five healthy plates every week
  • Include at least one serving of each of the five food groups on their plate (a healthy plate = all five food groups)
  • Log healthy plates on a designated challenge log sheet for 6 consecutive weeks (participants choose their time-frame of the challenge)

 DenverWellness Worksite Programs Minimize
Weight Watcher's

 

 

 “Freshstart” Smoking Cessation Program.

Register by February 28, 2012. Class starts on March 1st, at 12pm - 1pm

  • "Freshstart" smoking cessation programs, sponsored by the American Cancer Society, will be held at various City locations in 2012. 
  • The program consists of four classes, held every week from 12:00 pm - 1:00 pm.
  • All four classes must be attended and completed. 
  • The “Freshstart” smoking cessation program is free of charge.

Based upon the recommendations of your health assessment, employees can participate in a variety of online health programs, such as weight management, smoking cessation and exercise and nutrition programs.
