Basic Life and Accidental Death and Dismemberment
 
The City provides a basic life insurance policy on your behalf:

 Employee Class Coverage Amount
 Full-Time     2x annual salary, maximum $100,000
 
Three-Quarter & Half Time
(hired prior to Jan 1, 2002)   


2x annual salary, maximum $75,000
 
Three-Quarter Time
(hired after Jan 1, 2002)
 

1.5x annual salary, maximum $75,000
 
Half Time
(hired after Jan 1, 2002)
 

1x annual salary, maximum $50,000
 
Sheriff - Uniformed    
 
2x annual salary, maximum $400,000

The amount of your AD&D Insurance benefit is equal to the amount of your Basic Life Insurance benefit.  Enrollment is automatic; however, you should keep your beneficiary designation current.

Additional Life Plans
 
You may elect also Additional Life coverage for yourself and Spouse life coverage in units of $5,000 to a maximum benefit of $300,000.
If your are a new employee, you may elect up to $100,000 for yourself and $30,000 for your spouse if you enroll within 30 days of employment without being subject to medical underwriting approval.
 
If you missed your 30 day deadline or want more than $100,000 for yourself and $30,000 for your spouse, you must complete the Medical History statement ,  and may be required to complete a physical examination in order to receive additional coverage. 

You may also elect:
  • Additional Accidental Death and Dismemberment (AD&D) for yourself and your family in increments of $10,000 up to $500,000 at any time
  • $5,000 or $10,000 dependent life insurance policies for your children
